form if you wish to have your Pension payments deposited directly into your personal bank account.

Information Needed to Complete the Form
The following information is needed to complete this form:
- Member's name
- Member's Social Insurance Number
- Date
- Member's signature
- 'Void' blank cheque OR have your financial institution complete the second half of the form.
