Locked-In Agreement

Use
Use the Locked-In Agreement when you've terminated from the Pension Plan and are transferring out 'locked-in' pension funds that were not administered by Royal Trust.


Information Needed to Complete the Form
You must date the form and sign as the 'Annuitant'.



Notes
If your Pension funds are 'locked-in', those funds may only be transferred to a 'locked-in' RRSP. You may not take them as cash.

To withdraw your Pension funds, you must also complete the Application for Withdrawal of Pension Contributions form and the Direct Transfer of a Single Amount (T2151) form.

Questions on completing the form should be directed to the Plan Administrator.

Completed forms should be forwarded to the Plan Administrator.
