Use
Use a Pension Enrolment Card and Beneficiary Designation form to enrol in your
Pension Plan and to select your beneficiaries. Once you are a Member
of your Plan, all changes to your Member information or beneficiaries should also be
made by completing the form in full.
Since Pension Enrolment
Cards are specific to each Pension Plan, please contact the Plan
Administrator for a copy of your form.

Information Needed to Complete the Form
Your personal Member information is needed to complete the form.
Please follow the instructions on the form and ensure that you have completed the card in full.
If you have a spouse, you must designate him/her as your Pension beneficiary.
If you do not have a spouse you may select any person, or your estate, to be your Pension beneficiary.
Please ensure that you complete this section of the card.
